PROCEDURE

Contracted fingers may make daily activities difficult or impossible, including reaching into a pocket, shaking hands, putting on gloves, or even washing one’s face. Using a keyboard is particularly troublesome, both because of the inability to straighten the involved digit and the impaired reach of the small finger. Dr. Durand will take the time to carefully outline the various options available to you along with the benefits and risks associated with each procedure during your comprehensive one-on-one consultation.

For disease where it is not possible to undergo treatment with enzyme infiltration, the open approach leads to the highest permanent cure rates with minimal complications and is often the best course of action to restore hand function.

The surgery entails removing excess tissue in the palm of the hand. It is an intricate outpatient procedure that should be performed by an experienced hand surgeon under magnification, as care must be taken to protect the nearby blood vessels, nerves and tendons.

Although superficially this seems like a good idea because it is performed with minimal down-time, most experienced and certified hand surgeons will also tell you that this is the most dangerous procedure possible. The reason is because this is done without any visualization of associated anatomic structures: thus, there’s a very high level of associated tendon nerve and vessel injury in the hand with this technique.

Also, this technique has the highest incidence of recurrent disease of all treatments possible. The very high associated complication rate, and the highest incidence of recurrent disease make this procedure the least desirable of all treatment options.

HOW IS IT DONE

During a surgery an incision will be made along the natural creases of your hand – this helps to make scars less noticeable after surgery. Dr. Durand will then remove enough of the palmar fascia (the diseased tissue) to allow you to straighten your finger(s). In some cases, a skin graft may be needed if the skin surfaced has contracted so much so that the finger can’t relax as it should and if the palm can’t be stretched out flat.

This procedure is done as an outpatient basis under regional or local block anesthesia. Surgery typically lasts 30 to 40 minutes, after which a dressing is applied and the patient can return home.

POTENTIAL RISKS AND COMPLICATIONS

As with any surgery, there are risks associated with surgery for Dupuytren’s contracture. The likelihood of complications increases with the following:

  • The severity of the contracture
  • The number of contractures addressed in a single procedure
  • The presence of any other medical conditions

Dr. Durand will discuss each of the risks with you and will take specific measures to help avoid potential complications. Possible risks and complications of surgery for Dupuytren’s include:

  • Pain
  • Scarring
  • Injury to nerves and/or blood vessels
  • Wound infection
  • Stiffness
  • Loss of sensation. Temporary loss of sensation may result from stretching nerves that have been contracted for a long time.
  • Loss of viability of a finger/loss of a finger (rare)

BEFORE & AFTER

RESULTS

Most patients have improved movement in their fingers after surgery. However, because the condition is not “cured” with surgery, about 20 percent of patients will experience a meaningful degree of contracture recurrence. Additional surgery may be required for some patients.
