Pain at the base of the thumb can seem relatively minor until it begins interfering with ordinary tasks.

Opening a jar. Turning a key. Holding a cup. Pinching an object between the thumb and index finger. Using certain tools.

These movements have something in common: they place significant demands on the joint at the base of the thumb.

Arthritis of this joint — known as the thumb carpometacarpal or basal joint — is one of the conditions treated in Dr. Daniel Durand’s hand surgery practice.

Why is this small joint so important?

The thumb has remarkable mobility.

Unlike the other fingers, it can move in several directions and oppose them. This ability creates the precision pinch used in many everyday activities.

That mobility also places considerable stress on the basal joint.

As the cartilage within the joint deteriorates, movements requiring force and rotation can gradually become painful.

Early symptoms are not always constant

At first, pain may appear only during particular activities.

Someone may feel almost nothing at rest but experience sharp pain when trying to open a tight lid or operate a handle.

As the arthritis progresses, discomfort may occur more often. Pinch strength can decrease, and some patients begin unconsciously changing how they use the hand.

Tenderness or swelling may also develop around the base of the thumb.

Why does pinching become difficult?

Pinching an object between the thumb and index finger concentrates substantial force at the basal joint.

When that joint is arthritic, a task that seems light can therefore cause disproportionate pain.

This also explains why some people first notice the problem in the kitchen, at work or during manual activities rather than while resting.

Pain is not necessarily related to the size of the object. It depends more on the force and movement required to manipulate it.

Thumb pain is not always arthritis

Several conditions can cause pain around the thumb and wrist.

De Quervain’s tenosynovitis, for example, affects tendons on the thumb side of the wrist and may cause pain near that area. Dr. Durand’s practice also treats this condition and discusses surgical options when conservative treatment is insufficient.

Ligament injuries, trauma and other joint problems may also be involved.

The precise location of the pain, the movements that trigger it and an examination of the hand help distinguish among these possibilities.

Does thumb arthritis always require surgery?

No.

Treatment depends on the stage of arthritis, the severity of symptoms and their functional impact.

Arthritis visible on imaging does not automatically mean surgery is necessary. Conversely, pain that significantly limits use of the hand deserves assessment even when someone has grown accustomed to living with it for a long time.

Non-surgical options may be considered depending on the circumstances. When symptoms remain significant despite these measures, surgical options — including arthroplasty for certain indications — may be discussed.

The goal is to preserve a useful hand

For many patients, the main objective is not to make an arthritic image disappear from an X-ray.

They want to use the hand with less pain.

This distinction matters. Treatment should be selected according to hand function, pain and the person’s actual needs.

Someone who works with their hands every day may not face the same demands as someone whose activities place less stress on the thumb.
