Carpal Tunnel Syndrome: When Are Conservative Treatments No Longer Enough?

When are conservative carpal tunnel treatments no longer enough? Learn how symptoms and median nerve function guide treatment decisions in Montreal.

A diagnosis of carpal tunnel syndrome does not automatically lead to surgery.

When symptoms are mild or relatively recent, non-surgical options may be considered. Dr. Daniel Durand’s website mentions splinting, injection treatment and certain physiotherapy or occupational therapy approaches among the possible temporary treatments.

The question changes when symptoms persist, progress or begin affecting hand function.

At that point, the objective is no longer limited to controlling discomfort. It is important to understand the condition of the median nerve.

Why can a splint help at night?

Many people sleep with their wrists bent.

This position can increase pressure within the carpal tunnel and intensify symptoms. Keeping the wrist in a more neutral position may therefore reduce certain nighttime symptoms in selected patients.

This can help someone who wakes with tingling or numbness.

Improvement with a splint does not necessarily mean that the nerve compression has resolved.

A splint changes the position of the wrist; it does not create more anatomical space within the carpal tunnel.

What is the role of an injection?

An injection may be used in certain situations to reduce symptoms.

Its role depends on the clinical presentation and the purpose of treatment. For some patients, it may provide relief without an immediate need for surgery.

Symptom relief and definitive resolution of the compression, however, are not always the same thing.

If symptoms return or signs of nerve dysfunction appear, the treatment strategy needs to be reassessed.

Which symptoms are more concerning?

Nighttime numbness is common in carpal tunnel syndrome, but progression toward more constant symptoms deserves attention.

Someone may begin experiencing tingling during the day, have difficulty holding certain objects or notice reduced hand strength.

Carpal tunnel syndrome typically affects the area supplied by the median nerve, including the thumb, index finger and middle finger. Symptoms may also radiate into the forearm.

When numbness becomes persistent or weakness develops, the degree of compression should be evaluated.

Why does symptom duration matter?

A nerve that has been compressed for a long time is not necessarily in the same condition as one producing intermittent symptoms for only a few weeks.

Decompression aims to remove pressure from the median nerve. It cannot predict the exact speed or extent of recovery when the nerve has already sustained significant injury.

This is why the progression of symptoms matters as much as their intensity at a single point in time.

Simply waiting until the pain becomes intolerable is not always the best way to decide when to request an assessment.

When is carpal tunnel surgery discussed?

When conservative treatments are no longer sufficient or the degree of compression supports a surgical approach, carpal tunnel release may be considered.

Dr. Durand’s practice offers both open and endoscopic carpal tunnel release.

The objective is to reduce pressure on the median nerve.

The choice of approach depends on several factors and should be discussed in the context of the individual patient.
